As a former employer who once handled the medical insurance for my employees, I have to say this is a Really Bad Thing. Letting your employees go uncovered for three months without informing them is highly problematic. I'd certainly be asking a lot of questions of the boss. I'd also want to know why the insurance company didn't notify you of your coverage vanishing.
